Funnily enough, I used to clan with a player who only had one arm (and thus one hand) and he had all the controls on his mouse. He set up lots of aliases so that pressing mouse1 fired, but if you press mouse 2 then mouse1 it switched weapons. Strafe was mwheelup and down. He was actually pretty good as a scout. Not that great at Dming though.
